what are the plant species (a photo and a description would help if you don't know what they are)? what is the temperature and the pH? what is the lighting? Nitrates are fine nothing to worry about.

there was no need to do that. next time take them out of the pot and remove as much of the wool as you can. take off any dead leaves and trim dead roots then plant into the substrate.
if the same thing happens, take the plants out and trim off any dead leaves and roots, then re-plant them into the substrate.

what else do you keep in he tank jon? the thing is with some of the vallises is they either do or they dont grow in the tank. i had the same plant but died off over time when all others grew well. then a sword plant just died off after moving it in the tank. some plants can be just as sensitive as the fish.
